After your case is approved by the independent Immigration Attorney, Signature will send to your home a neatly prepared package ready to be filed via USPS Priority Mail® (at no extra cost) that includes:

• Form G-1145, E-Notification of Application;
• Form G-1450, Authorization for Credit Card Transaction (you can use this form if you want to pay the USCIS filing fees with a credit card);
• Form I-130, Petition for Alien Relative;
• Form I-130A, Supplemental Information for Spouse Beneficiary;
• Form I-131, Application for Travel Document, if applicable;
• Form I-485, Application to Register Permanent Residence or Adjust Status and Supplement A if necessary;
• Form I-693, Report of Medical Examination and a list of Doctors in your area  (you must take this form to the Authorized Civil Surgeon’s Office for completion at the time of your medical examination);
• Form I-765, Application for Employment Authorization;
• Form I-864, Affidavit of Support;
• Personalized cover letter to U.S. Citizenship and Immigration Services;
• Table of Contents and exhibit tab separators;
• Detailed but simple instructions on how to finalize the completion of your package and to mail your package to USCIS; and
• Mailing label with the correct USCIS mailing address to mail out your finalized package.

After you receive the package from Signature, you are responsible to mail the completed and signed forms, evidence, and required filing fees directly to USCIS. Signature DOES NOT do that for you.
